Could I be in a Mid-life crisis?

The Question\

I am 40 years old, never been married, never had a long serious relationship, don't date, failing to be successful at my job, always looked years younger than I am, lost hope for anything good.
I feel like I shouldn't exist, and would rather be dead.

You sound as though you are in a crisis, whether midlife, or just a life crisis. I urge you to see a psychotherapist in order to help work your way through your feelings to a more fulfilling life. I also suggest you take the New Warrior Training Adventure (The ManKind Project, which I believe can help you get a sense of mission and meaning in your life.
